Jeremy Moses, PT, DPT

Clinic Manager

Jeremy earned his Doctorate of Physical Therapy in 2010. The vast majority of his career has been spent treating patients in the outpatient setting, while also serving occasional home health patients. Jeremy has been with Millennium Therapy in Independence since the summer of 2010. He is always glad to assist and educate a patient and family to overcome the injury or illness that has brought them to therapy. As director of the Independence Clinic and Agency Administrator for four extension sites, Jeremy is very proud and honored to work with such caring and knowledgeable therapists as well as efficient and experienced office and support staff. In his spare time he enjoys spending time with family, friends, playing and watching sports, the outdoors and traveling.


Justine Reed, PT

Justine has been a licensed physical therapist since 2003. She is diversely experienced in multiple settings such as the outpatient clinic, nursing homes, assisted living facilities, home health and works with patients of all ages. Her love and greatest expertise is in the outpatient clinical setting where she feels her defined manual skills help her improve the quality of life for patients living with pain. She not only treats patients that have pain, but also treats balance disorders, vertigo, sports injuries, post-surgical, work injuries, and neuromuscular injuries. She became a certified lymphedema therapist in 2012. She has also received advanced training in pelvic floor dysfunction and is now seeing patients with pelvic health issues such as pelvic pain and incontinence. Justine’s passion is to help improve the quality of life for others by reducing pain, improving function, and limiting disabilities. Justine enjoys spending her free time with her family, being outdoors, and running.


Jennifer Trebon, PT, DPT

Jennifer Trebon graduated from Midwestern University with a Doctorate of Physical Therapy in 2008. Prior to joining the Millennium Therapy team in the summer of 2021, she practiced in multiple outpatient clinics in several areas of the US and has experience in treating spine and sacroiliac dysfunctions as well as non-operative and post-operative shoulders and knee joints. Neuro experience includes gait training, balance training and designing strength training programs for adults with developmental disabilities. Jennifer has also been trained in trigger point dry needling to help address musculoskeletal pain. Outside of work she enjoys spending time with her husband, two kids and two dogs.


Kelley Fischels, OTR/L

Kelley has been a proud member of the Independence Millennium Therapy team since the spring of 2004. She graduated with honors in occupational therapy from the University of Wisconsin – Milwaukee in 1992. She has worked in the OT field since 1992 and since 1996 she has worked strictly in rehab of the upper extremities. Her work experience includes 3-1/2 years at Mercy Medical Center in Cedar Rapids, and 10 years at the Physicians Clinic of Iowa – Hand Clinic in Cedar Rapids. Kelley has advanced skills in splinting and is able to efficiently rehabilitate shoulders, elbows, forearms, wrists, hands, and fingers. She treats minor to traumatic injuries, progressive diseases, and congenital deformities, helping people to restore their necessary ADL functions of work, play, and self-care. Conservative management is offered to prevent surgery, but she also sees a plethora of post-op diagnoses, having a mutual respect and great rapport with area orthopedic surgeons, rheumatology specialists, and family physicians. Diagnoses she commonly treats include cumulative trauma, arthritis, tendon and nerve injuries, burns, amputations, sprains, strains, dislocations, and fractures. Kelley enjoys living the country life at home on her farm with her husband, two daughters, and two Labradors. She enjoys quilting, exercising, knitting, and gardening.


Madison Elsbury, OTR/L

Madison has been a licensed occupational therapist since 2016. Since joining Millennium Therapy in June of 2016, she has gained experience in skilled nursing and long term nursing facilities, home health, hospital, and outpatient therapy of all ages. In April of 2018, she became a certified lymphedema therapist. Madison’s favorite thing about being an occupational therapist is the ability to improve individual’s quality of life by enhancing independence and safe participation in activities they love to do. In her free time she enjoys being outdoors, running, and biking.


Kayleen Turnis, SLP

Kayleen Turnis, SLPleen graduated with her Master’s Degree in May 2018 and began her career as a Speech-Language Pathologist with Millennium Therapy in June 2018. She has worked with patients ages 3-100+ with various diagnoses in the Independence outpatient clinic, home health, and several nursing home settings. She has treated patients whom have difficulties with speech sounds, language, social communication, voice, fluency, cognitive communication, feeding and swallowing. Her favorite aspect of her job is seeing her patients progress with their treatment goals while succeeding in their communication skills and ADLs. Kayleen enjoys spending time with her family, friends, cat-Leah, singing, reading, and catching up on her favorite television shows! 😊


Joan O’Loughlin-Good, PTA

Joan has been a Physical Therapist Assistant since 2002. The majority of her career has been spent treating patients in the outpatient setting, while also serving the community in patients’ homes. Joan has been with Millennium Therapy in Independence since the opening of the outpatient clinic in April 2004. She has experience in outpatient orthopedics and rehab, home health, and nursing home care. Joan loves all aspects of working at Millennium, especially the opportunity to treat a variety of injuries and ages in various settings in the rural community. In her spare time she enjoys spending time with her three children, husband and extended family, working outdoors, gardening, and hiking in the woods.


Tiffany Melancon, PTA

Tiffany has been with Millennium Therapy in Independence since 2010.  She has been practicing as a licensed physical therapist assistant since 2007. She has worked in a variety of settings including outpatient, nursing homes, assisted living, and home health. Tiffany received her certification in manual lymph drainage from the academy of lymphatic studies in 2007. She also has experience in muscle energy techniques to realign pelvis, sacral, and rib asymmetries. Tiffany loves to work with patients to help relieve pain, improve the body’s movement and limit disabilities. Tiffany enjoys spending time with family, antique furniture shopping, gardening, and going to car shows.


Ann Cooksley Ohl, Office Manager

Ann is the Office Manager at the Independence outpatient clinic. Her main responsibilities include patient billing, insurance verifications and accounts receivables. Ann has been with Millennium Therapy since the clinic opened in Independence in 2004. She enjoys boating and spending time with her family and friends.


Ashley McGraw, Administrative Assistant

Ashley joined the Independence Clinic in fall of 2020 as an Administrative Assistant. She is responsible for patient registration and scheduling, insurance verification, as well as other clinic and patient needs. She will graduate in spring of 2021 with a bachelor’s degree​ in management for health care services. Ashley enjoys reading books, boating, concerts, and anything that includes family time.


Krista Hintzman, Administrative Assistant

Krista has been on staff at the Independence Clinic since the summer of 2021 as a clinic administrative assistant. Some of her responsibilities include patient registration, insurance verifications, and helping out with various clinic and patient needs. Krista enjoys volunteering at her church, cooking with her husband, and quality time with family.
